SULLIVAN 180 PROGRAM:

Community Health Champions

SULLIVAN 180 Community Health Champions are local volunteers who commit to serve as healthy-living champions, coaches and leaders for friends, neighbors and colleagues ready to improve their health.

CHCs work in various areas to offer a grassroots, all-inclusive approach to health that unites the entire community, including those whose voice is often under represented. They have committed to promoting health and wellness in their community groups, neighborhoods, churches, schools, worksites, and/or civic organizations. A CHC volunteer can also form and lead a Sullivan Club 180, which can be any group that identifies its own healthy living activity running/walking groups, smoking cessation, meetings to share healthy meals and recipes, or starting a yoga class.

CHCs offer peer support and motivation for those who want to take proactive steps in improving their health and lives, and assist in various volunteer projects.

Sullivan 180 provides support and coordination for CHCs. Our Outreach Coordinators will help further the grassroots impact, by providing access to resources, information, networking opportunities and more!

Join the movement!

This innovative model is a way to help people adopt healthier behaviors that are meaningful and sustainable.
