SULLIVAN 180 PROGRAM:
COMMUNITY CHALLENGE – Learn, Commit & Act to Encourage Healthy Behaviors in Sullivan County!
As we look at our roadmap to better health, there are community-led solutions to encouraging healthy behaviors that can be implemented across our county. Knowing this, we have asked our Community Advisory Board members and other local organizations to commit to one or more strategies for the next year. These community-led solutions are supported by our CHIP, Sullivan County’s Community Health Improvement Plan and are evidence based strategies known to improve health outcomes.
During the course of the next year, we want our partners to utilize local resources and agencies that can help them stick to their commitments and reach our communal goal of encouraging healthy behaviors. You can learn more about the CHIP’s Goals, Strategies and Actions and their respective resources below, and see which organizations have made commitments to changing the health of Sullivan County, one degree and one strategy at a time.
